Preventive care aids in catching or stopping serious diseases and medical problems before the conditions become major problems. Some examples of preventive care include annual check-ups, immunizations, and flu shots, and other screenings or tests.

Our Membership Program
We provide individuals, who do not have insurance or have a high deductible health insurance plan, the options to participate in our Preventive Care Membership program. The program is also available to small-to-medium businesses who would like to provide their employees access to preventive care and urgent medical services. • Covers typical preventive primary care consultations including in office chronic disease screening and care coordination
• Include 15 visits per year for preventive and acute care management
• Virtual visits in the comfort of your home, via phone or video, when appropriate
• Coordination of care with your specialists, labs, and imaging referrals
• Discounted rate with our partnered pharmacies, labs and imaging centers.
• Timely and ongoing communications per HIPPA guidelines
• Same-day and next-day visits
• In-depth visits and consultations (30-60 minute appointments)

Membership Exclusion

• Additional charges for extensive management of chronic diseases including further testing and procedures
• Mental health counseling (Ask for our affordable self-pay therapy rates)
• Does not include visits to specialists and the hospital
• Does not count toward your insurance deductible, if you have one.
